For courses where I was instructor of record, I include the following in chronological order by semester:
MAT 221-M002, Summer 2018, complete course evaluation (distributed by Math Dept.)
MAT 295-M008, Spring 2019, complete course evaluation (distributed by Math Dept.)
MAT 221-M002, Summer 2020, partial course evaluation (Note: This was the first summer the Math Dept. transitioned to using EvaluationKIT for student course evaluations. I include the first two pages of this evaluation as they are more relevant to my teaching; the other survey questions involved use of technology etc.)
MAT 286-M002, Fall 2020, summary of quantitative data only (Note: There is no qualitative data for this semester as the written comments sections were not made available on EvaluationKIT by the Math Dept.)

The reason I include complete course evaluations from surveys distributed by the Math Dept. is because they are short and, in my opinion, easier to read. I include summaries of evaluations by EvaluationKIT as these surveys are much longer and contain survey questions that are not as relevant to my teaching. The overall survey questions by EvaluationKIT post Summer 2020 are more relevant to an instructor’s teaching. With this change, rather than include raw or partial data, I have chosen to include summarized data from student evaluations post Summer 2020 to keep my portfolio at a reasonable length. Written comments in the summaries are reproduced as written on the evaluations.
